I understand that the time it will take to repair my unit may vary. I agree to pay the diagnostic fee of $75 at the time i drop my unit off. I understand if I have electrical issues, the diagnostic fee will be an extra $150. If for any reason I do not want to move forward with the repairs recommended from the diagnostic, I understand that my unit WILL NOT BE REASSEMBLED.


I understand that after 3 days of being notified my bike is complete, I will be charged an additional $10 a day for storage fee. If F.T.B Mechanics is unable to reach me on the contact information, or my unit has accumulated excessive storage fees, after 30 days my unit will belong to F.T.B Mechanics LLC.


I hereby authorize the above repair to be done along with the necessary materials. You and your employees may operate above vehicle for purposes of testing, inspection, or delivery at my risk. An express mechanics lien acknowledged on above vehicle to secure the amount of repairs thereto. It is also understood that you will not be held responsible for loss or damage to vehicle or articles left in vehicle in case of fire, theft or any other cause beyond your control.
